In accordance with the Act respecting the protection of personal information in the private sector, Physio + Hamel is committed to ensuring the confidentiality of privacy and protecting the personal information of all its customers, users, suppliers, employees/workers and all other natural persons for whom personal information is provided by them and collected through our website, our records management systems, client portals, email, fax, in person or by telephone.

That you authorize the company Physio + Hamel, the employees/workers assigned in the performance of their duties and the conduct of activities in order to meet the obligations of care and services and to process your personal information according to their obligations towards your personal information and for the purposes of set out below.

Physio + Hamel ensures the security and confidentiality of data management by implementing strict and secure measures to protect your personal information, whether from data collection, to the use, disclosure, retention, modification or destruction of personal information of each individual mentioned above, while respecting the various legal terms and conditions regarding the protection of personal information.
Physio+ Hamel uses an ADDA Tech customer record system that covers all Bill 25 compliance requirements and more by implementing ISO 27001 and SOC2 certification. This includes clear policies and practices for the governance of personal information, as well as comprehensive management of privacy incidents and risks. ADDA Tech offers a complete Information Security Management System (ISMS) and Clinicmaster’s servers are located in Canada inside Microsoft Azure data centers.
Under the Privacy Policy, Physio+ Hamel and its employees/workers ensure compliance in their practice and are subject to the law on the protection of information arising from the patient-professional relationship. Access to clients’ personal and health information is strictly confidential and only used in the course of their duties.
No access to this information is authorized to any other person.
Anyone working at Physio + Hamel must comply with this policy and internal procedures. Any violation of this policy will result in dismissal.
To identify you when you interact with us through our various sources of communication, this information may be used and collected by Physio + Hamel and assigned employee/worker personnel.
Whether it is medical or other data, it is treated securely in accordance with applicable privacy laws and regulations.
RETENTION PERIOD OF PERSONAL INFORMATION
Physio+ Hamel retains personal information for the period prescribed by the Privacy Act. When this period has expired, personal information will be destroyed or anonymized in a secure manner.

PRIVACY AND DE-INDEXATION COMPLAINT PROCESS
If you believe that your personal information has been used in a non-compliant manner, or that a privacy incident involving personal information has occurred, please notify us immediately in order to begin the complaint process as set out in this Policy.
If applicable and reasonable, during this process, you will be asked to provide us with your contact information, the nature of your complaint and provide us with sufficient details so that it can be properly assessed.
A period of 30 days following receipt of your justifications will be applied to allow us to proceed with the various steps described below:
- Receive the complaint;
- Register the complaint in a complaint tracking system;
- Review the complaint and determine whether human rights under the law have been violated;
- Investigate to determine and understand the circumstances of the incident, review internal procedures with the individual(s) involved;
- Take the necessary steps to resolve the complaint (changes in procedures, staff training, removal of certain information from search results);
- Communicate with the person concerned, inform him or her of the results of the investigation and the steps taken to resolve the complaint in order to maintain trust and openness;
- Follow up to ensure that the actions taken are effective and that the complaint has been satisfactorily resolved
CONSENT
At Physio+ Hamel, all personal information collected is from the person concerned and with his or her consent. We will not dispose of your personal information to third parties in any way.
However, personal information may be collected without the consent of the individual concerned from a third party if authorized by law.

- Informed (precise, rigorous and specific);

In accordance with the Act respecting the protection of personal information in the private sector, Physio+ Hamel appoints Ms. Manon Hamel, administrative assistant as the person responsible for data security and the protection of personal information in the company from an employee/worker, a client, a user, a supplier and any other natural persons for whom personal information is provided.
Role is responsibility of the person responsible for the protection of personal information within the company:
- Implement strict measures to ensure the protection of personal information and privacy;
- Ensure compliance with current laws;
- Oversee and ensure compliance with practices and procedures to protect personal information;
- Updates and train employees/workers regarding responsibilities and security of personal information;
- Respond to requests for access, rectification, cessation of dissemination and de-indexation;
- Approving governance rules for personal information;
- Keep a record of disclosures of personal information without the consent of the individual concerned, including in the event of a confidentiality incident;
- Be consulted when assessing the risk of harm being caused to a person whose personal information is affected by a confidentiality incident;
- Take reasonable steps to reduce the risk of harm to affected individuals and prevent the recurrence of the incident.
